Request a Verification of Certification Letter
Use this request form to send a verification of your current Texas registration or certification to another state dental board in the United States. This form is not to be used for checking the status of an application for a registration or certification.
- Complete a Request for License or Registration Verification Form. Verification letters can be mailed to the licensee or another State Board.
- Pay required fee.
- If you require the TSBDE to fill out a State Board Licensure Certification/Verification Form from another state, please include this form in your mailing.
We do not release information about licensees to third parties without permission from the licensee. Please have the licensee send an email to licensinghelp@tsbde.texas.gov to authorize the TSBDE to release information.
