Request an Update to Dentist Specialty Practice Type
Dentists interested in updating their licensing record online to reflect their specialty practice should mail or e-mail a copy of their ADA CODA-accredited specialty education residency certificate of completion to the TSBDE. Specialty recognition is not printed on the Registration Certificate.
Only the licensee may submit an update to their licensing record maintained by the TSBDE.
All requests should include the following:
- Licensee’s Full Name
- Last 4 of the Social Security Number
- Address and Phone Number
- Current License Number
- Specialty Certificate of Completion
