Annual Renewal of Mobile Dental Facility & Portable Dental Unit Permit

Please follow these steps to renew:

1. A Mobile Dental Facility or Portable Dental Unit Owner is required to complete a renewal form annually by December 31st.   
2. Renewal of the registration can be completed by mail within 45 days prior to the expiration date. 
3. You may request that a paper renewal application be mailed to you here. Include in the subject line, “License Renewal.”
4. Mail the renewal form to the Texas State Board of Dental Examiners (TSBDE) at the address listed at the top of the application. An Incomplete Renewal Form will be returned without action.
