On or after September 1, 2019, a permit holder may not administer sedation/anesthesia under a level 2, level 3, or level 4 permit to a high-risk or pediatric patient unless the permit holder has completed the requirements and has received authorization from the Board to administer sedation/anesthesia to high-risk or pediatric patients.

Should you need this authorization added to your license, please submit the application following the instructions below. Upon Board approval, a letter will be mailed out for your records and at your next renewal the endorsement will be reflected on your renewal certificate.

There is no additional fee for this endorsement.

Online Instructions

Please click on this link to navigate to the virtual system to begin with the online application.

  • Login with User ID (email) and Password.
  • From the Quick Start Menu → under, “Manage your license information” you will need to choose your application. Select from the drop-down menu, “Add Sedation (SHRP & SPP)” and then click on “SELECT” to be taken to the introduction page to proceed.
  • You will have the option to update your public address, home, and work addresses.
  • After you have entered your information an application summary will be available for review.
  • Be sure to have your course provider email your official course completion certificate/letter to anesthesiainfo@tsbde.texas.gov for review.
  • Upon submission of the application request, the application will be processed within 7-10 business days. A letter confirming the approval of the Sedation of Pediatric and/or Sedation of High Risk will be sent via your online licensing account. TSBDE no longer issues renewal certificates; you may print your updated renewal certificate from your online account.
  • If the application status is “Open” and the application has been reviewed, then the user can select “Details” to view the deficiencies the anesthesia program specialist has entered at the time the application was reviewed. This information will be visible from the “Quick Start Menu” under “View Application Status.”
